Oregon School Immunization EXCLUSIONS 2021
To attend any school - whether public, private, charter or alternative school in Oregon students must be up to date with their immunizations. Shots are also required for attendance at all preschools, Head Start programs, and certified child care facilities.
Oregon requires immunization against eleven vaccine-preventable diseases like:
- Diphtheria
- Tetanus •
- Pertussis (whooping cough)
- Polio • Varicella (chickenpox)
- Measles
- Mumps
- Rubella
- Hepatitis B
- Hepatitis A
- Hib – only for children under 5 years of age
All students must be up to date and have current records to their school or facility by February 17th, 2021.
